Medical records are confidential and are protected by state and federal laws. Although the medical record is the property of San Antonio Community Hospital, the patient has the right to access information contained in their medical record. You may obtain a copy of the information in your medical record by signing an authorization to release the information. There is a fee for copies.

To obtain more information on how to receive a copy of your medical record, please contact Health Information Management weekdays, from 8:00 a.m. to 4:30 p.m., at 909.920.4750. In order to expedite your request print out and complete the Authorization to Use and Disclose Health Information form. This request can also be faxed (909.920.4745) or mailed to the Health Information Management Department for processing.
